APGov - Info Needed for Field Trip

This is the information that I need for our special field trip.  Be sure to bring it with you to class on Thursday:

• last name, first name, middle initial
• Date of birth
• Social Security number
• Are you a U.S. citizen?
• Nation of birth
• Gender
• City and state of residence
